How to Sign Up with MyChart
As a patient of Hoag, you will have access to your medical records and notes all through MyChart. After you complete the sign-up process, you’ll be able to access all the benefits MyChart has to offer.
Benefits of MyChart
MyChart offers patients personalized and secure on-line access to portions of their medical records. Communicate with your doctor, access your test results, request prescription refills and manage your appointments. You can also use MyChart to learn about your care team, referrals, request services, and access educational information and benefits.

Now that you’re a Medicare Advantage member with Hoag, you’re just one step away creating the healthy lifestyle that’s best for you. After answering just a few simple questions, you’ll have a customized roadmap to help you start managing your health care journey as a Medicare Advantage and Hoag patient.
What is an Annual Health Assessment?
The Annual Health Assessment is a comprehensive visit that is covered under your insurance plan. During this visit, you will meet a Nurse Practitioner who works closely with your primary care provider as part of your care team at Hoag.
Together, you will develop a personalized plan and road map to help you stay healthy. Medicare Advantage plans provide coverage for annual assessments.
ORIGINAL MEDICARE
What do I need to do during Open Enrollment?
You are expected to begin Medicare on the first day of the month that you turn age 65, which may fall outside of the typical Open Enrollment period. You should receive your Medicare card in the mail 3 months before you turn age 65.
If you did not receive your card, it’s important for you to call the Social Security Office at (800) 772-1213.
