Documents to Bring
The following documents are required to complete your tests:
- Lab order form
- Photo ID (driver license, passport, school ID, or other government issued ID accepted)
- Health insurance card
Fasting
Certain tests require fasting. The most common ones are the lipid panel, glucose, cholesterol, and triglycerides. Contact your ordering physician for specific instructions.
