At Hoag, recovery follows a seamless continuum of care designed to meet each patient’s evolving clinical needs. Treatment begins with a hospital-based Detox Program, where patients undergo safe, medically managed detoxification with 24-hour access to hospital medical resources.
Following detox, patients transition into the SolMar Residential Treatment program, receiving intensive medical and therapeutic care in a structured, healing environment. As progress is achieved, individuals move into the Partial Hospitalization Program (PHP) for comprehensive therapy and medical support while beginning to reintegrate into daily life. The final step, the Intensive Outpatient Program (IOP), offers a flexible yet structured schedule that sustains therapeutic engagement, accountability, and connection as patients return to work, school, and family life.
Inpatient Medical Detox and Rehabilitation
Medical Detoxification (Detox) Detox is the first step in recovery for patients who need medical support to safely withdraw from alcohol, opioids, or other substances. Patients are monitored around the clock by a team of physicians, nurses, and behavioral health specialists. Because withdrawal can sometimes trigger serious health complications, such as seizures, cardiac issues, or exacerbation of other medical conditions, our hospital-based detox program is equipped to address both the physical and psychiatric needs of patients. Medications may be used to ease withdrawal symptoms, minimize cravings, and ensure safety. Our Medically-Assisted Treatment (MAT) program helps to alleviate the brain's cravings without producing the intense high or deadly respiratory depression that drugs may cause. Emotional support and counseling are also introduced to help patients begin their recovery journey.

Detox addresses the physical dependence on substances. Once stabilized, patients move into therapies that focus on the psychological and behavioral aspects of addiction.
SolMar Residential Program
Solmar offers Orange County’s only hospital-based residential program, providing 30 to 45 days of structured, medically supervised care within a world-class hospital setting. Under the leadership of a four-time Board-Certified Medical Director, Matthew Reed, MD, MSPH, our program integrates expert medical oversight with compassionate, patient-centered treatment. Collaboration between physicians, licensed therapists, nurse practitioners, registered nurses, and certified counselors ensures each patient receives comprehensive, coordinated care that addresses both medical and psychological needs.
Our hospital-based model allows immediate access to hospital-grade medical resources for co-occurring conditions and emergencies, creating the safest environment for patients with complex medical histories, ongoing health concerns, or elevated clinical needs. Each treatment plan is customized to address substance use disorders, mental health conditions, and comorbid medical concerns, guided by evidence-based practices and ongoing clinical assessment.
Therapeutic interventions include Dialectical Behavior Therapy (DBT) and Cognitive Behavioral Therapy (CBT), complemented by experiential modalities such as equine-assisted therapy, art therapy, and music therapy. This integrated approach supports emotional regulation, insight, and self-efficacy while incorporating the foundational structure of the 12-Step recovery philosophy to promote lasting change.
Specialized programming is available for older adults, addressing age-related health challenges and fostering recovery through tailored interventions that support physical and cognitive wellness. As patients progress, they transition seamlessly into outpatient and step-down levels of care, ensuring continuity, stability, and sustained recovery beyond the residential setting.

Outpatient Programs (PHP and IOP)

Presented at AABH’s 56th Annual Training Conference in April, this prestigious honor reflects our commitment to innovation, clinical integrity, and exceptional patient outcomes.
The programs were recognized for their individualized phased-treatment model, seamless continuity across all levels of care, and evidence-based therapeutic approaches that promote lasting recovery.
Together, the Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) provide structured, medically integrated treatment designed to support patients as they transition through recovery, offering the consistency, accountability, and compassionate care necessary for long-term wellness.
Partial Hospitalization Program
The Partial Hospitalization Program (PHP) represents the next step in the continuum of care following residential treatment. It provides a structured, supportive environment where patients continue intensive therapy while beginning to practice greater independence. PHP bridges the gap between 24-hour residential treatment and fully outpatient care, offering both clinical consistency and opportunities for real-world reintegration.
Our PHP meets five days per week for six hours each day, providing a comprehensive schedule that includes group therapy, individual sessions, family involvement, and continuous medical and psychiatric oversight. Patients maintain direct access to their physician, psychiatrist, and nursing team for medication management and ongoing assessment, ensuring continuity of treatment and safety throughout the recovery process.
The program follows a phased model of care tailored to each patient’s needs. Rather than adhering to fixed timelines, each phase is defined by measurable goals that reflect clinical progress, skill mastery, and readiness for sustained recovery. This individualized approach allows patients to advance at a pace that builds confidence, accountability, and lasting wellness.
Intensive Outpatient Program (IOP)
The Intensive Outpatient Program offers structured therapy and clinical support while allowing patients to maintain work, school, and family responsibilities. Participants attend therapy sessions three days a week for 3 hours each day, receiving individualized care through a combination of group therapy, one-on-one therapy, and education focused on relapse prevention and practical life skills.
As the final phase in treatment, IOP provides patients the opportunity to solidify what they have learned, strengthen coping strategies, and plan for the future. This level of care promotes accountability, consistency, and community connection—helping individuals maintain sobriety, reinforce healthy routines, rebuild balance in their daily lives, and continue progressing with confidence and support.
