What can I expect on my first visit?

Arrive at your appointment time 15-30 minutes early with your completed health questionnaire (can be downloaded from website), Driver’s license, insurance card, and physician prescription. You will be seen for the initial evaluation. The therapist will discuss your medical history, current problems or complaints, and your goals. The therapist will perform a comprehensive evaluation and examination. A personalized treatment program will be developed based on the information determined at the time of the evaluation. During the first session you may receive some form of exercises or education to assist with your recovery.

What should I wear?

Wear loose clothing that will allow access to the particular body part that will be treated.

What insurance will you accept?

By participating in nearly every major health insurance plan and network, Hoag makes it easy to afford the finest medical treatment available. Your insurance company may be affiliated with one of the plans listed on the Hoag Hospital website. If you have questions about your health insurance, please call our Insurance Information Service at 800/400-HOAG (4624) or e-mail Insurance@hoag.org.

It is each patient’s responsibility to inquire about his or her own specific insurance benefits. If you have questions regarding your coverage, please contact your employer’s benefits department or your insurance company.

Do I need a physician referral before I can be treated at Hoag?

Yes, the state of California requires a physician referral for treatment.

How many times will I need to come for treatment?

The number of visits is variable based on your diagnosis, the treatment plan established at the time of the evaluation and the therapist will establish a treatment plan based on diagnosis, severity of impairments and functional limitations.

How long will each session last?

The initial evaluation is 40-45 minutes. Subsequent visits will range from 30-45 minutes depending on your needs on that particular visit.

What does Hoag Rehab department offer over other Rehab practices in Orange County? (Why should I come to Hoag?)

The Rehabilitation Services at Hoag Hospital offers individual treatments by a licensed therapist, with a master’s or doctorate in their given field. Many of our therapists have received advanced training in a variety of fields such as neurology, orthopedics, vestibular rehabilitation, hand rehabilitation, lymphedema, communication, voice, and swallowing. The therapists are in communication with the physician to develop the proper treatment plan for each patient. Hoag Rehabilitation Services offers post therapy individualized aftercare programs for patients in a supervised setting.

What do I do if I need to cancel or change my appointment?

Please call if you need to change or cancel your appointment, as soon as possible to allow us to fill scheduled appointment time. Two consecutive “No Show” will result in termination of services.
