Community Nurse Navigation (CNN)

The Community Nurse Navigation (CNN) Program was created to help meet the health care needs of low-income, uninsured, and underinsured patients. Community Nurse Navigators focus on prevention and management of chronic diseases and support patients in accessing care by navigating through health care barriers. Our highly trained nurses help individuals and families address access to care issues through care coordination, resource brokering and health advocacy. Nurses serve as a compassionate guide, providing health coaching and support through each step of the care continuum. Our nurses also focus on health literacy, creating health and wellness workshops and programming to empower individuals to improve their health. Nurses provide competent bicultural and bilingual services in English and Spanish.
