Meet the Team
The Hoag Margolis Family Inflammatory Bowel Disease (IBD) Program is led
by gastroenterologist Caroline Hwang, M.D. who specializes in the treatment
of individuals with crohn's disease and ulcerative colitis. Together
with Hoag gastroenterologists, a clinical nurse navigator and support
teams, the Orange County community can access multidisciplinary care for
inflammatory bowel disease.
Program Director
Caroline Hwang, M.D.
Dr. Caroline Hwang is the Program Director for the Margolis Family Inflammatory
Bowel Disease (IBD) Program at Hoag. Dr. Hwang received her Doctor of
Medicine from the David Geffen School of Medicine at the University of
California Los Angeles. She then went on to complete an internal medicine
residency at Columbia University Medical Center in New York, a clinical
research fellowship for the Hereditary Colon/Pancreatic Cancer Prevention
Program at Columbia University and a gastroenterology fellowship at University
of California San Francisco. Dr. Hwang is board certified in gastroenterology
and is a recognized expert in the field of inflammatory bowel disease.
Dr. Hwang joined Hoag following her time at Keck Medicine of USC, where
she was Medical Director of their IBD Center and served as an assistant
professor of medicine, as well as a mentor for numerous physicians-in-training
specializing in IBD/GI care. She has focused research on the role of nutrition
in IBD outcomes and responses to therapies, as well as disparities in
IBD outcomes.
Dr. Hwang is a member of the American Gastroenterological Association,
American College of Gastroenterology and the Crohn’s & Colitis
Foundation.
Clinical Nurse Navigator
Jamie Vincent, BSN, RN, CMRSN
Jamie Vincent, BSN, RN, CMRSN is the IBD Clinical Nurse Navigator that
provides coordination of care across the continuum for patients with Inflammatory
Bowel Disease. The IBD clinical nurse navigatorIn her role she assists
in enhancing the patient experience and improving patient access to care.

Registered Dietitian Nutritionst, Margolis Family Inflammatory Bowel Disease
(IBD) Program
Chelsea Carroll, RDN

Chelsea is a Registered Dietitian with a focus in gastrointestinal conditions/diseases,
more specifically Inflammatory Bowel Disease (IBD), Crohn's Disease
and Ulcerative Colitis, Celiac Disease, and Irritable Bowel Syndrome (IBS).
As the IBD RDN, Chelsea helps patients identify their trigger foods, provides
specific nutrition recommendations individualized to each patient's
needs, helps prevent and correct nutrient deficiencies, and monitors overall
nutrition status of her patients.
As the IBD Registered Dietitian Nutritionist, Chelsea provides:
- One-on-one nutrition counseling where the RDN and the patient develop an
individualized nutrition care plan
- Group nutrition education classes where patients can feel comfortable discussing
general questions and/or concerns with the RDN and their peers
- Collaboration with the multi-disciplinary team to deliver the best nutrition
care to all patients
