With the recent launch of Hoag at Home, experienced doctors are making house calls, offering comprehensive, high-quality and personalized care to patients in the comfort of their homes.

Hoag at Home represents an evolution in Hoag’s commitment to the community and a recognition of the changing demographics of the county. People are living longer, healthier lives, and overwhelmingly they want to age in place. Hoag at Home allows them to do that.

“We want patients to have the same world-class experience that they receive if they visit their doctor’s office, one of our health centers, or Hoag hospital,” said Jeff Hammond, executive director of Hoag at Home.

“When one of our providers visits a patients’ place of residence, that patient knows they will have the seamless continuity of care they expect from Hoag.”

Patients often recover faster from injury or surgery at home, surrounded by family. Through Hoag at Home, medical and support services are brought to the patient and each patient receives an individual care plan to meet their specific goals and needs, said Joomo Yang, M.D., who, along with Michael Haga M.D., recently became members of Hoag Medical Group and bring more than 20 years’ experience each in-home care for aging patients.

“I was drawn to in-home care because I find that I’m better able to meet my patients’ needs at the home,” Yang said. “I have found that patients prefer to be treated at home, and our services allow them to do that.”

The physicians anticipate great demand and are planning expanded services to meet the growing need. Additional services coming soon to Hoag at Home include private duty home care, home infusion services, and home medical equipment.

“We have the technology and expertise to support patients in ways that were once only possible in a hospital setting,” Haga said. “This means we can coordinate with a patient’s entire medical team, while taking the time to slow down and really listen.”
