GI Foregut Fellowship
Overview
As one of a select group of American Foregut Society-approved pilot fellowship programs, the Hoag GI Foregut Fellowship is designed to train the next generation of leaders in foregut disease through immersive, multidisciplinary education and high-volume clinical experience. Fellows will work alongside nationally recognized experts in advanced therapeutic endoscopy, foregut surgery, esophageal motility, reflux disease, Barrett’s esophagus, and endobariatric therapy, gaining expertise across the full spectrum of medical, endoscopic, and surgical management of complex foregut disorders. Supported by robust procedural volume, dedicated mentorship, and a collaborative foregut ecosystem, fellows will develop the clinical judgment, technical proficiency, and scholarly foundation necessary to practice independently and lead comprehensive GI foregut programs. Upon completion, graduates will be uniquely positioned to serve as regional and national experts in foregut disease, advance innovation in the field, and help shape the future standards of foregut care.

Program Details
Procedure Variety and Volume
Hoag's GI Foregut Fellowship offers fellows exposure to a broad and diverse spectrum of advanced foregut and endobariatric procedures. The pilot program is designed to help establish training volume thresholds and competency standards for future GI foregut fellowship programs nationwide. Fellows can expect extensive hands-on experience and progressive responsibility in the performance and interpretation of the following procedures:
Suture-based endoscopic bariatric procedures (ESG, TORe, etc.)
Per-oral endoscopic myotomy (POEM) including myotomy of cricophayngeus, esophagus andpylorus
Transoral incisionless fundoplication (TIF)
Endoscopic mucosal resection of dysplastic Barrett's esophagus
Primary interpretation of esophageal manometry
Primary interpretation of ambulatory wireless pH monitoring
Primary interpretation of catheter-based pH/impedance monitoring
Primary performance and interpretation of impedance planimetry
Salary, Vacation, and Professional Development
The fellow will be compensated at the appropriate PGY salary level.

Travel accommodations and reimbursements are granted for the fellow to attend American Foregut Society Annual Meeting.
Available Positions: There is one (1) position available for the 2027-2028 fellowship year.
Program Length: 1 year (July 1st – June 30th)
Schedule, Call, & Coverage
The GI Foregut Fellowship curriculum will be delivered through a structured, competency-based educational model that integrates apprenticeship-style learning with formal didactics and multidisciplinary instruction. Fellows train directly under high-volume faculty in foregut surgery and advanced endoscopy ensuring ample procedural exposure. Training includes three days per week in advanced endoscopy/OR, a weekly half-day clinic, and a weekly half-day devoted to interpretation of HRM, FLIP, reflux testing, and foregut imaging consistent with AFS curriculum structure.
Research
Fellows will participate in research opportunities provided by the research team of the Digestive Health Institute. They will be expected to produce a national presentation, manuscript, book chapter or hospital quality initiative during their fellowship year.
Educational Conference Schedule
Formal educational components include weekly didactic lectures, case-based conferences, monthly multidisciplinary team meetings, video-based learning modules, and access to institutional textbooks and digital resources. Fellows receive curated foregut-specific content, including video libraries of Hoag procedures, AFS/ASGE educational materials, and evidence-based guideline reviews.

Candidate Qualifications
Applicants must be board eligible and/or board certified in Internal Medicine, and completed a Gastroenterology Fellowship program.
Applicants must be eligible to obtain an unrestricted California medical license prior to the beginning of fellowship training. We are unable to provide J-1 or H-1B visa sponsorship at this time. ****
