Patients and family members should request a transfer through your physician or assigned case manager.
To expedite your transfer request, please call (949) 764-8163 and be prepared to provide the following information:
Patient’s full name
Patient’s date of birth
Current patient status (Inpatient or Emergency Department)
Diagnosis
Requested specialty and/or service
Reason for transfer
Level of care required (ICU, Stepdown, Telemetry, Med/Surg)
Referring hospital name
Case manager’s name and contact number
Referring physician’s name and contact number
Referring facility fax number
Clinical staff at the referring facility call (949) 764-8163, available 24/7 to initiate the transfer request.
Provide the face sheet and clinicals (if available).
Speak with the accepting specialist or hospitalist as needed.
Hoag case management assesses appropriateness and coordinates with on-call physicians.
Upon physician acceptance, we will secure an inpatient bed at the appropriate level of care and notify the referring party to arrange transportation.

Note: Patients and families should request a referral through their physician or assigned case manager.
Hoag complies with EMTALA, evaluates and stabilizes emergency conditions before transfer, and coordinates per federal regulations.
All shared patient information is handled per HIPAA throughout the transfer process.
