    Triage — three paths to transfer

    Information to have ready before you call

    To expedite your transfer request, please call (949) 764-8163 and be prepared to provide the following information:

    • Patient’s full name

    • Patient’s date of birth

    • Current patient status (Inpatient or Emergency Department)

    • Diagnosis

    • Requested specialty and/or service

    • Reason for transfer

    • Level of care required (ICU, Stepdown, Telemetry, Med/Surg)

    • Referring hospital name

    • Case manager’s name and contact number

    • Referring physician’s name and contact number

    • Referring facility fax number

    5-Step Transfer Process

    Clinical staff at the referring facility call (949) 764-8163, available 24/7 to initiate the transfer request.

    Share patient info

    Provide the face sheet and clinicals (if available).

    Specialist consult

    Speak with the accepting specialist or hospitalist as needed.

    Review & coordination

    Hoag case management assesses appropriateness and coordinates with on-call physicians.

    Acceptance & bed assignment

    Upon physician acceptance, we will secure an inpatient bed at the appropriate level of care and notify the referring party to arrange transportation.

    Neuro Receiving Services

    Advanced neurological receiving services for time-sensitive and complex neuro cases including stroke management, neuro ICU care, and neurosurgical coverage. Designed to support rapid evaluation and transfer coordination for critical patients.

    ECMO Receiving Services

    Advanced ECMO (Extracorporeal Membrane Oxygenation) receiving capability for critically ill patients requiring advanced life support. Supports complex transfer coordination and specialized critical care teams.

    Referrals may be initiated by:

    • Case managers

    • Insurance representatives

    • Hospital staff

    • Physicians and medical directors
