Hoag On-Demand is available for adults ages 18 and older in the state of California. You must be physically located in California at the time of your visit.
We accept most PPO insurance plans including Aetna, Anthem, Blue of California, Cigna, HealthNet, Medicare, TriCare, UMR, and United Healthcare. During account setup, you can verify if your specific plan is covered. If you have questions about coverage, email ondemand@hoag.org for help verifying your benefits.
Your copay, or amount owed if you have a deductible, will be determined during the insurance verification process and payment will be collected prior to your visit.
If you don't have insurance or your plan is not accepted, we offer a self-pay rate of $182 per visit. Payment will be collected prior to your visit.
Our clinical care team can evaluate and treat a range of common conditions during your virtual visit including:
If you require lab tests, imaging, or other diagnostic services, the Hoag On-Demand Care Coordinator will arrange an appointment through Hoag or refer you to the appropriate facilities.
We cannot treat life-threatening emergencies, severe injuries, or conditions requiring immediate in-person evaluation. If you are experiencing an emergency, please call 911 or visit your nearest emergency room.
Our providers cannot prescribe controlled substances or manage refills for medications prescribed by another medical provider. Only medications recommended by your provider during your visit can be prescribed. If you need a primary care provider, please visit the Hoag Medical Group site or send a note to ondemand@hoag.org and we will be happy to assist you.
You will need 3-5 minutes to complete a short intake. During intake, have a valid ID for identity verification (Driver’s license, passport, or other government issued ID) and your insurance card with your membership number and group ID available. Payment for the visit will be collected prior to joining the virtual waiting room and visiting with a provider. Most patients connect with a provider within minutes of completing their intake.
Hoag On-Demand can be accessed from the browser on your mobile device or computer. Please ensure you have a stable internet connection and are in a quiet, private space with limited distractions.

The providers are board certified Hoag Nurse Practitioners who embody Hoag's standard of excellence when caring for patients.
Your Nurse Practitioner will review your intake information, discuss your symptoms, may ask you to show affected areas on video, and develop a treatment plan. You'll receive a summary of your visit and any prescriptions or recommendations after the visit concludes.
If you need labs, imaging, other diagnostics, a referral, or follow-up appointment with a primary care provider, a Care Coordinator will assist you.
Visits can last up to 15 minutes, though the duration depends on your specific needs and concerns.
If your provider refers you to a Hoag specialist or recommends a follow up visit with a Primary Care Provider, a Care Coordinator will reach out to assist you. Our Care Coordinators manage appointment scheduling and all coordination of your next Hoag visit.
