FAQ: Hoag’s Contract with Scan Health Plan
The contract between Hoag and Scan Health Plan for Medicare Advantage HMO members will expire on Dec. 31, 2026. Beginning Jan. 1, 2027, Hoag providers, services and facilities will no longer be in-network for Scan Medicare Advantage HMO members. We know this change may be concerning. Read on for answers to frequently asked questions, including information about continuity of care and options for maintaining access to Hoag.
The contract between Hoag Memorial Hospital Presbyterian (“Hoag”) and Scan Health Plan (“Scan”) for Medicare Advantage HMO members will expire on Dec. 31, 2026. Beginning Jan. 1, 2027, Hoag will no longer be in-network for Scan Medicare Advantage HMO members.
During October and November 2026, Scan will assign you to a new primary care physician (PCP) who is not part of Hoag.
Beginning Jan. 1, 2027, you will no longer have access through your Scan Medicare Advantage HMO plan to Hoag specialists or elective inpatient and outpatient services at Hoag health care facilities, including:
Hoag Hospital Newport Beach and Hoag Hospital Irvine
Hoag Orthopedic Institute
Hoag Medical Group
Hoag Physician Partners
Hoag Specialty Clinic
Hoag Health Wellness Centers
Hoag Spine Center
Hoag Women’s Health Institute
Hoag Breast Center
Hoag Family Cancer Institute
Hoag Digestive Health Institute
Yes. This change will not affect your access to emergency medical treatment at Hoag Hospitals. Emergency care is always considered in-network and will not be impacted by this change. If you or a family member experiences a medical emergency, please visit the nearest emergency department.
If you are receiving care from a Hoag health care provider, you may have the right to continue seeing your provider for a designated period under “Continuity of Care” provisions, as determined by Scan.
Contact Scan’s customer service department using the number on the back of your member ID card to request continuity of care. Eligibility is determined by Scan.
If maintaining your current Hoag primary care provider and continued access to Hoag specialists and facilities is important to you, Hoag participates in Original Medicare and the following Medicare Advantage HMO plans:

Continuity of Care
If you are receiving care from a Hoag healthcare provider, you may have the right to continue seeing your provider for a designated period after Hoag becomes out-of-network under Continuity of Care provisions. Eligibility for Continuity of Care is determined by Scan.
