• EpicCare & TapestryLink User Agreement

This document is a confidentiality and acceptable use agreement for individuals requesting to have access to Hoag’s EpicCare Link and/or Epic Tapestry systems. By initialing each section and signing the form, the user confirms that they understand and agree to follow Hoag and HIPAA privacy and security rules when handling patient information.

• Site Administrator Responsibilities*

This document is a Site Administrator Responsibilities Agreement for Hoag Software, including EpicCare Link and Epic TapestryLink. It outlines the duties and responsibilities of individuals designated as Site Administrator(s) who manage user access for office staff and employees.

* Site Administrator(s) play a key role in keeping access organized, secure, and aligned with employees’ job duties.

* If the new group is requesting a new user to be a site administrator, this form needs to be filled out and included with the completed EpicCare & TapestryLink User Agreement for the site administrator.
