Hoag Interventional Radiology Pioneers “X-Ray Vision” Augmented Reality Technology, Marking a West Coast and Global First

IRVINE, Calif. (June 13, 2025) – Hoag is transforming the future of interventional radiology procedures by becoming the first hospital on the West Coast and the first non-academic hospital in the world to offer MediView’s XR90 Augmented Reality (AR) 3D Procedural Navigation System. This innovative “X-ray vision” technology provides clinicians with unprecedented real-time visualization and precision during image-guided procedures.

In their first cases, Hoag also achieved two additional global firsts: (1) the use of MediView’s AR navigation in a minor procedure room versus a traditional angiography suite or operating room setting, and (2) using the XR90 navigation system for a complex spleen procedure. This breakthrough technology enhances precise execution of minimally invasive, CT- and ultrasound image-guided procedures.

Patients are commonly referred to interventional radiologists for minimally invasive procedures for a wide variety of medical conditions. Traditional 2D flat screen displays used for these procedures present challenges related to image quality and ergonomics. MediView’s system uses a powerful computer system and imaging headset display to convert complex CT imaging data into detailed 3D images that are then fused with live ultrasound imaging.

The new technology enables Hoag’s interventional radiologists to clearly visualize internal organs and disease pathology aligned with the patient’s anatomy beneath their skin, providing full depth and detail for unprecedented 3D “X-ray vision.” The clinician’s interventional tools can also be tracked in real-time, with a light ray displaying needle trajectory throughout placement and manipulation. The enhanced 3D visualization and tracking capabilities have the potential to make procedures faster while maintaining precision.

“This marks a significant milestone for Hoag’s interventional radiology team and heralds the profound impact that AR-guided navigation will have on patient care,” said Alexander Misono, M.D., M.B.A., R.P.V.I., Chief of Interventional Radiology at Hoag’s Sun Family Campus in Irvine.

The physical setting during the use of MediView’s XR90 system in these cases was transformational, as it moved procedures from a traditional interventional angiographic suite to a minor procedure room. This demonstrates that the system has far broader clinical utility beyond interventional radiology procedures and can be leveraged in a variety of settings. By doing this, it can expand access to more patients and procedures with enhanced workflow at a lower cost.

“This new technology represents one of the most significant advancements in ultrasound-guided interventional radiology in more than a decade,” Dr. Misono said. “Through its generous support of advancing technological innovation at Hoag, the Hoag Hospital Foundation continues to uphold our mission to provide the highest standard of care to our community.”

The technology was funded by Hoag Innovators, a group of entrepreneurs, community leaders, and philanthropists, who drive innovation at Hoag through strategic collective giving and collaborative philanthropic investment. Thousands of CT- and ultrasound-guided procedures are performed every year by Hoag’s interventional radiology team, and an increasing number of patients require these procedures. Today’s physicians and patients increasingly require access to smaller, harder-to-reach, and more technically difficult targets for biopsies and minimally invasive interventions.

Hoag has implemented the 3D AR visualization and navigation ultrasound system at Hoag’s Sun Family Campus in Irvine and has provided comprehensive training for physicians and clinical staff.

“This new technology furthers Hoag’s position as a leader in interventional radiology and opens new possibilities for research, education and industry partnerships,” said Scott Norton, Director of Radiology at Hoag. “We are thrilled to continue to bring cutting-edge treatment options to our patients.”
