Hoag On-Demand is available for California residents ages 18 and older, and to California residents ages 12 to 17 when a parent or legal guardian is present for the visit. You must be physically located in California at the time of your visit.
We accept most PPO insurance plans including Aetna, Anthem, Blue of California, Cigna, HealthNet, Medicare, TriCare, UMR, and United Healthcare. During account setup, you can verify if your specific plan is covered. If you have questions about coverage, email ondemand@hoag.org for help verifying your benefits.
Your copay, or amount owed if you have a deductible, will be determined during the insurance verification process and payment will be collected prior to your visit.
If you don't have insurance or your plan is not accepted, we offer a self-pay rate of $182 per visit. Payment will be collected prior to your visit.
If you require lab tests, imaging, or other diagnostic services, the Hoag On-Demand Care Coordinator will arrange an appointment through Hoag or refer you to the appropriate facilities.
We cannot treat life-threatening emergencies, severe injuries, or conditions requiring immediate in-person evaluation. If you are experiencing an emergency, please call 911 or visit your nearest emergency room.
Our providers cannot prescribe controlled substances or manage refills for medications prescribed by another medical provider. Only medications recommended by your provider during your visit can be prescribed. If you need a primary care provider, please visit the Hoag Medical Group site or send a note to ondemand@hoag.org and we will be happy to assist you.
