Whether you have just been diagnosed with diabetes or have been living with it for a while, you may feel overwhelmed or unprepared to understand what it means for how you live day-to-day. The Allen Diabetes Center is here to help!

Diabetes education is a recognized part of your diabetes care and is covered by Medicare and most health insurance plans when it is offered through an accredited diabetes education program, which ic certified by the American Diabetes Association. As a member of your health care team, a Hoag Diabetes Educator makes managing your diabetes easier. They work with you to complete your education and develop a plan to stay healthy, and give you the tools and ongoing support to make that plan a regular part of your life.

Initial Assessment: ABCs of Diabetes (90 minutes, 1:1 session)

Class 1: Self-Management Strategies – Nutrition and Physical Activity (90 minutes, group session)

  • Meal planning
  • Carbohydrate counting
  • Nutrition labels
  • Being active and staying active
  • Review goal setting

Class 2: Tools for Managing Diabetes (90 minutes, group session)

  • Tools to monitor
  • Medications
  • Risk reduction
  • Avoiding setbacks

Class 3: Living Well with Diabetes (90 minutes, group session)

  • Dining out
  • Traveling with diabetes
  • Sick day guidelines
  • Managing stress

Physician referral required to participate in this program.
