Based on this information, she will refer you to the most appropriate provider and help you schedule an appointment with them. There are a few ways to connect with our dedicated nurse navigator:
Web Form – Fill out and submit the form above and our nurse navigator will call or email you as soon as possible.
Phone Call – Call our nurse navigator directly at 949-764-1411. We are available Monday – Friday between 9 a.m. to 5 p.m. Pacific time. If no one answers, please leave a message with your name, phone number and a brief description of your situation. We will do our best to return your call as quickly as possible.
Depending on your situation, you may be referred to one of several types of providers: neurosurgery, orthopedics, pain management, rehab, etc. Our nurse navigator will help you determine which is best.
