Important Update About Hoag's Patient Portal

As of January 1, 2016, the online health summary for discharged inpatients for Hoag Hospital Newport Beach and Hoag Hospital Irvine have transitioned to a new patient portal. If you had previously registered to use our old patient portal or if you would like to begin using our patient portal for the first time, please visit this website to register.

Accessing Your Online Health Summary

For patients hospitalized at Hoag, an online health summary will be available within 36 hours after discharge.

Your patient health summary will include the following:

  • Date and location of admission and discharge
  • Reason for hospitalization
  • Any surgical procedures performed
  • Names of Hoag doctors who provided your care
  • Health problems identified
  • Medications prescribed
  • Medication allergies identified
  • Vital signs at discharge
  • Blood test results available at time of discharge that we are permitted to share electronically under California State Law
  • Name of other care providers (if applicable)
  • A customized Patient Care Plan, including goals and instructions
  • Discharge instructions
  • Online health summaries for hospitalization(s) began after 4/1/2014

Other Ways to Request Your Medical Records

To obtain medical records copies, a signed authorization is required from the patient or authorized representative*.

*Authorized representative includes parent of a minor patient (under 18 years of age), legal guardian, or is named in the patient’s Advance Health Care Directive as agent to make health care decisions. Proof of representation will be required before records will be released.

If you are unable to download and print the authorization, a written request is acceptable. There is specific information that is required on all requests.

Hours of Operation: Monday through Friday, 8 a.m. until 5 p.m.

Hoag charges patients a reasonable fee for copies of medical records. Patient requests that are 25 pages or less will remain free of charge. A fee schedule can be provided upon request.

Turnaround Time for Requests

Due to the large volume of requests that are received daily, your records request will be processed within 5-7 business days. Every effort is made to accommodate walk-in requests during normal business hours.

Records will be mailed to where you specify on your request. If you wish to pick up your records, please note that on your request and provide a telephone number where you can be reached. Our Release of Information staff will call you when your records are available for you to pick up.

Picking up Copies of Medical Records

Pick up times: Monday - Friday (Closed holidays), 8:00 a.m. – 4:30 p.m.

If you plan to pick up copies of your medical records, you must provide photo identification (state drivers license, state ID, passport, or military ID’s are forms of valid photo identifications) in order to verify we are releasing the information to the correct individual.

If you are unable to pick up your own records and someone else is picking them up for you, you must provide a note giving Hoag Hospital permission to release your information to that person. The individual must then provide their own photo identification at time of pick-up.
