Richard Afable, M.D., M.P.H.
President and Chief Executive Officer

Richard Afable, M.D., M.P.H., is President and Chief Executive Officer of Hoag Memorial Hospital Presbyterian.
Formerly, Dr. Afable was the Executive Vice President and Chief Medical Officer at Catholic Health East, an East Coast health system comprised of 31 acute care hospitals and 47 long-term care facilities. Before joining Catholic Health East, Dr. Afable was the founder and President/CEO of Preferred Physician Partners (PPP), an Ohio-based physician practice management company that supported physician groups and provider networks. He has also served as an Associate Professor of Medicine at Wake Forest University from 1991 to 1996 and as Clinical Assistant Professor of Medicine at Northwestern University from 1983 to 1991. Early in his career, Dr. Afable spent ten years in private practice in Chicago, specializing in internal medicine and geriatrics.
Dr. Afable received his BS degree from Loyola University in Chicago and an MD from the Loyola Stritch School of Medicine. He obtained his MPH degree from the University of Illinois School of Public Health and a certificate in business administration from Villanova University.
Flynn Andrizzi, Ph.D.
Sr. Vice President Development, Hoag Memorial Hospital Presbyterian
President, Hoag Hospital Foundation

Dr. Andrizzi joins Hoag with over 20 years of experience in fundraising and marketing.
Over the past 15 years, he worked for several large universities including serving as the chief development officer at a large public university's foundation. Dr. Andrizzi also participated in the planning and implementation of several comprehensive multi-million-dollar campaigns, including the successful completion of a university's $1 billion campaign.
Alan Beyer, M.D., F.A.C.S.
Executive Medical Director, Hoag Orthopedic Services

Alan H. Beyer, M.D., F.A.C.S. is a board-certified orthopedic surgeon specializing in sports medicine, arthroscopic surgery of the knee and total knee replacement. Since his involvement with Hoag Orthopedic Services began in 1983, he has been a pivotal member and is currently the medical director for this Center of Excellence, which is consistently rated in the top percentile of the nation for orthopedic surgery.
Dr. Beyer graduated from Georgetown University School of Medicine. During his residency at the Hospital for Joint Diseases in New York, he became the Chief Resident of Orthopedics and completed a sports medicine fellowship at the Kerlan-Jobe Orthopedic Clinic. His passion for sports influenced his decision to pursue this specialty, and thus excel in a vocation where preserving an active lifestyle is a key focus.
Throughout the years, Dr. Beyer has created numerous papers and presentations on sports medicine and joint replacement. He is also currently involved with clinical research as the principal investigator for two studies.
Dr. Beyer generously lends his services to many organizations—he's the team physician for University High School in Irvine and is both the chairman and team physician for the Newport Beach Breakers. The recipient of the 2007 Vin Jorgenson Award for volunteerism at Hoag, Dr. Beyer remains an active volunteer at the hospital and its events. He is a past president of Hoag's 552 Club, and enjoys serving on various Hoag fundraising committees for the annual Toshiba Golf Classic and Newport Beach Breakers Tennis.
Jan L. Blue
Vice President, Human Resources

Jan L. Blue joined us in November 2008 as Vice President of Human Resources. Prior to Hoag, she was Senior Vice President & Corporate Officer at FCG, Inc., a leading provider of outsourcing, consulting, systems implementation and integration services and proprietary software products for healthcare, pharmaceutical, and other life sciences organizations throughout North America, Europe and Asia. Ms. Blue was with FCG for 10 years until they were sold to Computer Sciences Corporation in a public trade.
Before joining FCG, she was a healthcare consultant at the Hay Group from December 1996 through December 1998. Additionally, she served in a variety of senior HR leadership positions at Presbyterian Healthcare System in Dallas (now Texas Healthcare Resources) for 10 years. Ms. Blue is also a Registered Nurse.
Currently, she serves as a board member for the 552 Club,which is affiliated with Hoag Hospital's Foundation.
Ms. Blue attended Texas Tech University and has a BSN from West Texas A&M University.
Robert Braithwaite
Chief Operating Officer

As Chief Operating Officer, Robert has senior operating responsibility for both campuses of Hoag Hospital. He is currently the interim CEO of Hoag Orthopedic Institute until a permanent leader is recruited to the position. Robert has been the chief administrative officer of Hoag Hospital Irvine, leading the process of developing, building, staffing and licensing both Hoag Hospital Irvine as well as Hoag Orthopedic Institute.
Mr. Braithwaite was previously employed at Hoag Hospital from 1992 to 1996 and from 1999 to 2007. He held positions as Administrative Resident, Purchasing Manager, Director of Materials Management, Assistant Vice President, Vice President, Support Services, Vice President of Operations and Senior Vice President, Hospital Operations.
A graduate of Brigham Young University, Mr. Braithwaite received his bachelor’s degree in health management and promotion and his master’s degree in health services administration from Arizona State University. He currently belongs to the American College of Healthcare Executives. In the past, Mr. Braithwaite has served in many community capacities including, SOS Clinic, South County Chamber of Commerce and several youth-related sports organizations. He is currently serving as a board member of the Irvine Chamber of Commerce. As a native Southern Californian, Mr. Braithwaite enjoys several outdoor related activities including landscape photography, golf and surfing.
Michael Brant-Zawadzki, M.D.
Executive Medical Director of Hoag Neurosciences

Dr. Brant-Zawadzki earned his undergraduate degree from Stanford University, and his medical degree from the University of Cincinnati College of Medicine, where he received the Hoffheimer prize, which is awarded to the top graduating medical student.
He trained in radiology and neuroradiology at Stanford University Medical Center, after which he spent six years on the Neuroradiology faculty at the University of California San Francisco, in the Neuroradiology Section. He has authored and co-authored more than 180 papers in professional journals. He has also authored or co-authored 17 textbooks and has a large number of book chapters to his credits.
Dr. Brant-Zawadzki lectures nationally and internationally. He is a consultant to industry in the imaging, pharmaceutical, and informatic fields. He is a member of multiple professional societies and sat on the Executive Committee of the International Society of Magnetic Resonance Medicine, as well as on the Task Force on Clinical Guidelines for the North American Spine Society. He serves on the editorial board of numerous professional publications including JAMA (Journal of the American Medical Association) and the American Journal of Neuroradiology. In 1989, he was elected to the Fellowship of the American College of Radiology and in 1993 he received the special Gold Medal from the Society of Magnetic Resonance in Medicine for outstanding pioneering achievements in magnetic resonance imaging.
Dr. Brant-Zawadzki served as medical director of radiology at Hoag Hospital and holds an Adjunct Clinical Professorship in Neuroradiology at Stanford University. He has recently been appointed as executive medical director of Neurosciences at Hoag.
Terri Cammarano, JD
Vice President and Chief Legal Counsel

Terri Cammarano, JD, is Vice President and Chief Legal Counsel at Hoag Memorial Hospital Presbyterian. Ms. Cammarano has been named one of "Fifty Women of Distinction" by her undergraduate alma matter, California State University, Fullerton. Ms. Cammarano received the President's Volunteer Service Award for 2007, presented by President Bush's Council on Service and Civic Participation.
Ms. Cammarano is a graduate of Loyola Law School, Loyola Marymount University, Los Angeles (J.D. and LL.M., Taxation), and California State University, Fullerton (B.A., with honors). She is admitted to practice in California and Hawaii, and before the United States District Court (Central District, California), the Ninth Circuit Court of Appeals, and the United States Tax Court.
Jack L. Cox, M.D., M.M.M.
Senior Vice President and Chief Quality Officer

Jack L. Cox, M.D., M.M.M., joined Hoag Hospital as chief quality officer (CQO) in September 2006. Dr. Cox is an industry expert who has been widely published and is a regular speaker in the areas of wellness, medical management, quality improvement, physician engagement and product evaluation. As a senior physician executive, Dr. Cox brings over 20 years of experience in medical practice, clinical performance improvement and corporate leadership in high-level regional and national positions.
As the senior executive directly responsible for all aspects of quality of care, measurement, performance improvement and patient safety, Dr. Cox works with Hoag medical staff, senior administrative staff and Board of Directors to lead and direct hospital resources, and will develop and oversee a comprehensive, effective quality management and performance improvement program for the hospital.
Prior to joining Hoag, Dr. Cox served as group vice president and chief medical officer of Premier, Inc., a nation-wide health care alliance of more than 1500 not-for-profit hospitals, specializing in group purchasing, supply chain management, clinical performance improvement and health care informatics. Serving with Premier since 1999, Dr. Cox played a significant role in leading Premier to become the largest national group purchasing organization in 2005 with a purchasing volume of more than $27 billion. Many quality initiatives of national prominence have been initiated under Dr. Cox's leadership, including laying the foundation for the Premier / Centers for Medicare & Medicaid (CMS) pay-for-performance demonstration project for hospitals.
Dr. Cox received his medical doctorate from University of Mississippi School of Medicine and holds an active medical license in Utah, Mississippi and North Carolina. He attained his masters degree in medical management from Tulane University School of Public Health in 1998.
Robert Dillman, M.D.
Executive Medical Director, Hoag Institute for Research and Education

Robert O. Dillman, M.D. is the Executive Medical and Scientific Director of Hoag Cancer Center in Newport Beach, CA, where he has been since 1989. Prior training and experience included a BA from Stanford, MD and internal medicine residency and chief residency at Baylor College of Medicine, and hematology and oncology fellowship at the University of California San Diego (UCSD). He is board certified in internal medicine, medical oncology, and hematology. Before coming to Hoag, Dr. Dillman was Assistant Director of the UCSD Cancer Center and Chief of Hematology/Oncology at the San Diego VA Medical Center, then Director of Experimental Clinical Oncology and Associate Director of the Ida M. Green Cancer Center of Scripps Clinic and Research Foundation in La Jolla, California.
Since coming to Orange County in 1989 he has been a Clinical Professor of Medicine at UC Irvine. Dr. Dillman has authored or co-authored more than 270 scientific articles, reviews, book chapters, and monographs. He is recognized internationally for his work in lung cancer and cancer biotherapy, including monoclonal antibodies, interleukin-2, adoptive cellular therapy and autologous cancer cell vaccines. Dr. Dillman is a highly regarded medical educator, having delivered more than 700 invited lectures throughout the US and in 10 foreign countries.
He delivered the Plenary Address entitled Monoclonal Antibodies in the Treatment of Human Malignancy at the 11th International Conference on Human Antibodies and Hybridomas held in Dublin, Ireland in 2004.
Dr. Dillman served as Chairman of the Cancer Biotherapy Research Group (1990-1999), President of Orange County American Cancer Society (1991-1993) President of the Inter-American Society for Chemotherapy (1997-1999), and President of the International Society for Biological Therapy of Cancer (2000-2002). He has chaired or co-chaired 15 national or international educational symposia.
From 1992-2008 Dr. Dillman has been listed in every edition of the Best Doctors in America, and in 1992 became the first hematologist or oncologist in Orange County, California to be so selected by peers. In addition, he has been named as a "Top Doctor," "Top Physician," "Best Doctor," and "Physician of Excellence" in Orange County, California. In 2006 he was named "Orange County Physician of the Year" by the Orange County Medical Association.
During his tenure as Medical Director of Hoag Cancer Center, the five-year relative survival rate for Hoag patients with invasive cancer has improved from 58 percent to 76 percent, and improved much more rapidly than has occurred nationally.
Richard Martin, MSN, RN
Senior Vice President, Clinical Operations and Chief Nursing Officer

Richard Martin has been Vice President of Patient Care Services and Chief Nursing Officer at Hoag Hospital since 1992. In this role, Mr. Martin oversees all nursing departments, Hoag Women's Health Services, as well as a variety of patient care services, including the emergency care unit, perioperative services, pharmacy and infection control.
He previously served as Assistant Vice President of Specialty Care for Hoag Hospital. Prior to joining Hoag Hospital, Mr. Martin was Assistant Vice President of Patient Care Services at HCA Lewis-Gale Hospital in Salem, Virginia.
Mr. Martin is active on numerous nursing committees and boards, several of which were formed in response to the state and national nursing shortage. In the late 1990s, he was appointed to the Scott Commission, a panel of nursing industry leaders, to address the nursing shortage in California. He also serves on the VHA National Nursing Leadership Council, the Orange County Nurse Executive Council, and on the Advisory Board for Nurseweek. At California State University, Long Beach, Mr. Martin is on the Advisory Board and Adjunct Faculty for the School of Nursing.
Additionally, Mr. Martin participates in several professional organizations including: American College of Healthcare Executives, Association of California Nurse Leaders, American Organization of Nurse Executives, National League of Nurses, American Society for Quality and Leadership Tomorrow. He is also an active community member, serving as Administrative Representative on the Board of Directors for the Share Our Selves Clinic.
Throughout the years, Mr. Martin's work has brought him much recognition. In 1984, he received the Nursing Excellence Award from Humana St. Luke's Hospital. The International Honor Society awarded him Sigma Theta Tau in 1985, and in 1986 he was recognized as Who's Who in American Nursing. California State University, Long Beach honored Mr. Martin with the Spirit of Nursing Award in 1995 and the Distinguished Nursing Leader Award in 2003.
Jennifer Mitzner
Senior Vice President and CFO

As Senior Vice President and CFO, Jennifer Mitzner oversees all hospital accounting, including general accounting, payroll, accounts payable, and treasury for Hoag Hospital. She also holds responsibility for the finance department, corporate compliance, corporate communications, human resources, revenue cycle and legal department.
Ms. Mitzner, who joined Hoag in 1994, participates on a number of internal committees, including the Finance and Audit Committee of the Board of Directors, the Investment Management Committee of the Board of Directors and the Hoag Foundation Finance and Audit Committee.
Prior to joining Hoag, Ms. Mitzner worked as a CPA with KPMG Peat Marwick focusing mainly on healthcare and insurance industry clients on both audit and consulting engagements.
A certified public accountant, she received her bachelor's degree in accounting from Texas Christian University and her master's degree in public administration/healthcare administration from the University of San Francisco.
Timothy C.L. Moore
Senior Vice President and Chief Information Officer

Mr. Moore is the Senior Vice President and Chief Information Officer of Hoag Memorial Hospital Presbyterian in Newport Beach, California. In this role, Mr. Moore will lead the overall Information Technology at Hoag Hospital.
Mr. Moore has 21 years of IT experience in healthcare. Before joining Hoag, his most recent experience was at Perot Systems, which is a global provider of IT and business solutions to healthcare, government, manufacturing, banking, insurance, and other industry clients. Tim served as an Executive Director for Provider Healthcare at Perot, where he worked with large health care systems and was a champion in the pursuit of health care delivery transformation. Prior to Perot, Tim was Vice President and CIO at Carondelet Health System, which is a national health care system with 19 facilities. Tim's experience in health care is not only in IT. He has also held positions as a registered nurse and a pharmacy assistant.
Mr. Moore earned his bachelor in science degree from Louisiana State University Medical Center, School of Nursing. He was a business major with an accounting minor at the University of Nevada, Las Vegas.
Cynthia Harris Perazzo
Senior Vice President, Strategy and Business Development

Cynthia Harris Perazzo has an extensive background in strategic business development and the healthcare industry. Before joining Hoag as SVP of Strategy and Business Development she served as Vice President of Corporate Development for Premier, Inc. Her responsibilities included supporting strategy development and deployment and business development for Premier. She joined Premier in 2000 as the Vice President of Strategic Initiatives responsible for identifying and executing strategic investments and business development.
Cynthia serves on the Board of Trustees for the National Multiple Sclerosis Society Pacific South Coast Chapter. She holds a Master's degree in Business Administration from the Harvard Business School and a Bachelor's degree in Business Administration in Finance from the University of Texas at Austin.
Jim Rice
Vice President, Facilities & Operations

Jim Rice, FACHE, is Vice President of Facilities & Operations at Hoag Memorial Hospital Presbyterian. In this role, Mr. Rice oversees all facilities-related operations including engineering, biomedical services, communications, security, safety, environmental services, support services, and food and nutrition services. Additionally, he is responsible for clinical laboratory and pathology laboratory.
Mr. Rice previously was responsible for Hoag Orthopedic Services, one of the centers of Excellence, which includes the JointWorks Program, the Institute of the Spine and rehabilitation services. He assisted in the development of the Hoag Orthopedic Institute.
Mr. Rice joined Hoag in 1999 as Director of Materials Services and was promoted to Assistant Vice President of Support Services before his current position. He is a Fellow at American College of Healthcare Executives and was a member of their regent advisory council for the Southern California region from 2002-2011.
Before working at Hoag, Mr. Rice worked as associate administrator for an Orange County Hospital and has been employed in the healthcare industry for 20 years.
He received his bachelor's degree of science in health policy and administration from Pennsylvania State University (Penn State) and his masters of business administration at Pepperdine University. Mr. Rice also volunteers his time for Penn State University as a recruiting volunteer.
Sanford L. Smith, AIA
Senior Vice President, Real Estate and Facilities

In his role as Senior Vice President of Real Estate and Facilities, Sanford Smith heads the departments responsible for the planning, development and operations of all facilities projects for Hoag Memorial Hospital Presbyterian. The portfolio consists of approximately 2 million square feet and includes a diverse range of facility types, including the hospital, outpatient services and medical office buildings.
Prior to joining Hoag, Sanford was the Corporate Manager of Real Estate and Facilities for Toyota Motor Sales, USA, Inc. In the 18 years he spent overseeing Toyota's North American expansion, his department's leading edge work on environmental issues was recognized with many awards, including: the Global Innovators Award for Corporate Real Estate as well as honors from US Green Building Council, American Institute of Architects, Cal EPA, IFMA, and IIDA. Toyota's Real Estate and Facilities Department also received the Environmental Leadership Award from California's Governor, Arnold Schwarzenegger in both 2003 and 2007.
He is an active member of the Partner's Circle of the College of Environmental Design at Cal Poly Pomona. Sanford belongs to the American Institute of Architects and is a Director of Ability First (formerly Crippled Children's Society) and Chairman of their real estate committee.
Sanford received the prestigious Corporate Real Estate Leadership Award from the editors of Site Selection Magazine and was honored as the Corporate Real Estate Executive of the Year by the Los Angeles Chapter of CoreNet.
